I can get into the game perfectly fine, everything works, but despite having:
JIP LN NVSE
Stewie Tweaks
kNVSE
JohnnyGuitarNVSE
NVTF
NV Heap Replacer
NVAC
I still receive random and severe ticks and it seems that the combination of all of these did absolutely nothing.
Here are some examples:
Lag: Running through Springvale to Megaton
No Lag: Running in a straight line to Megaton
Lag: Looking at the nuke in Megaton (This is by far the worst, with my frames starting at 60 and dropping anywhere from 14 to 30, with the latter having extreme stutter and lag)
No Lag: Looking literally anywhere else in Megaton
Lag: First hallway of Rivet City Upper Deck (Also severe, dropping to an extremely laggy 30ish)
No Lag: Taking the most straightforward route to Rivet City along the river (Includes several firefights with SM, explosions, etc. etc.)
Additionally I have:
MCM (With Bugfix 2)
UIO
Just Assorted Mods
FOV Slider
Pip-Boy 2000 Mk VI
I assume none of these are related as the problem has persisted before and after the installation of these.
Any help or suggestions would be greatly appreciated.

I'm having this issue too. It normally happens to me after a couple hours of play, so maybe a ram thing?

What is your load order AND your rig's hardware spec.
I note that you are describing two areas with heavy graphical demands. Well, not exactly heavy but pretty taxing for weak rigs.
Looking at the nuke generally mean there's a lot of NPC moving around in your vision.
And rivet city is always taxing thanks to tight space dis-synergy with the "talk if pass by other NPC" behaviors. Upper Deck's Weatherly hotel is a hotbed of complaints from day one with no solution.
