This is my first time playing TTW, I want to try the game vanilla before I mess it up with mods and everything is working fine but when I start the game it plays the FO3 intro but then I wake up in Doc Mitchell's. I've been trying to find a solution for this all morning, I've been through the FAQ, searched to see if anyone else has a fix for the same problem and tried various solutions but none have worked. My load order is correct, the .ini is fine, script extender is completely fine.
Can someone tell me what I'm doing wrong or point me in the right direction, thanks

I had a very similar looking problem, but it was solved after reinstalling of Windows and startig to work under admin account (viewtopic.php?f=18&t=8547). But I would recommend you to check ttw_nvse.log (as jlf65 pointed viewtopic.php?f=18&t=8547#p69895) to verify is this is the same issue I had.

Thanks for the reply hopefully I can try to get a fix for this now,
There is no ttw_nvse.log0 file and the regular ttw_nvse.log file doesn't have any of the plugins inside, instead just has a [-1] in it.
I suppose I'll have to try and reinstall windows if there's no other fix
