Whenever I start a new game, it starts me in Doc Mitchells house.
Installed correctly, load order seems to be okay, new main menu appears, but i get vanilla start.

ttw_nvse.dll must be missing or deleted by your anti-malware product
also make sure you update to 3.2.1 and 3.2.2 to have the selection menu

I'm also having this exact same issue. I ran the TTW installer and applied both 3.2.1 and 3.2.2 patches. I had the installer save the files to it's own folder for MO2 to use.
Running the game through MO2 with TTW and JIP LN NVSE Plugin enabled as well as with a sorted plugin list still as directed by the FAQ, I'm still waking up at doc mitchells with no prompt of any kind other than new vegas dlc's loading in.
I looked at my TTW folder and it looks like it has everything it needs. The ttw_nvse.dll was in there as well. I am at a loss for why this isn't working the way it is. I know that nvse is working because the getnvseversion function in the console still works.

Ok. It looks like I fixed the problem I was having. It looks like it was a Mod Organizer problem. Switching from another game and then back to NV then changing my fallout.ini to what a user on this forum had seemed to fix it. Now when I launch the game, I get the TTW main menu background and I can start a new game in the CW or the Mojave.
