Don't know what you mean. All I can say is that regardless of anything I change, whether they be ini settings, disabling NVSR or ENBoost, running in fullscreen or windowed mode, or trying to roll back my Nvidia or Realtek drivers, I will still encounter the audio lag. It is completely baffling at this point. Maybe for some reason Gamebryo hates Nvidia 1000 series cards and higher. Or there is some other setting I am unaware of.

Roy is asking what is the same between your laptop and desktop? Stuff like same software, hardware, Operating System, etc. :)

Ok Roy, do you mind if I send you a save file where I can reliably reproduce the audio bug?

And they both run Windows 10, the desktop uses 1809 while the laptop uses 1803. They both have nvidia cards, the laptop uses 1060 while the desktop uses 1080, neither are TI. They both run the latest nvidia graphics and physx drivers, but rolling them back doesn't seem to help at all. It also doesn't matter if I don't have physx installed. I don't have nvidia audio drivers on either of them. They both have 16 gb ram. The TTW profile on both of them is exactly the same as well as the New Vegas installation. The audio lag issue is the same on both of them in terms of how random it is and how it seems to occur more often when explosions occur (i.e. car explosions). Other than that, I can't really think of anything else they have in common. I would have thought that the fact they use different processors may make a difference but apparently not.

EDIT: They both run Avast and Malwarebytes, they both have exclusions added for the New Vegas directory, the MO2 directory, and the mod profile directory for TTW

Well I know someone who has similar hardware to my desktop and also runs Avast, and he doesn't have any audio lag. Though I haven't asked him to test my save out yet. Also I tried disabling Avast shields and it didn't get rid of the audio lag.

Anyway I just submitted Google drive links to my save file to Roy, hopefully they will help shed some light on the issue.

One thing I forgot to mention is I tend to use VATS for almost every single gun shot I make when I am in combat, especially with my sawed off shotgun. Could that also have an effect on triggering the audio lag?
