Well I originally had a 1080 when I had this problem. Maybe a motherboard or PSU issue? or CPU? I'm going to reinstall windows and see if it helps.

If you are using this mod https://www.nexusmods.com/newvegas/mods/65094 uninstall it.

I never used that mod. Nor did I have any Nvidia audio drivers.

I just reinstalled Windows 10 and it seemed to only worsen the problem, now I get it whenever I load it up and immediately enter combat. Could it be one of the microsoft visual c++ redistributables causing a problem? Or maybe faulty ram?

Ok so I have mitigated the issue significantly by setting bManageFPS to 1 in NVSR. It doesn't completely remove the lag but now I can go in combat and reload the save for much longer than originally. I will test with new RAM and see if that helps. Also, since I reinstalled Windows and then reinstalled new vegas, I have been getting this strange issue where on my normal New Vegas profile, there are some missing sound effects, mainly my character doesn't make any noise when hurt and some gun impact sounds don't work. Yet when I switch over to the TTW profile all of the sounds work correctly. Could it be some kind of settings issue or file issue? The ini files for both profiles should be the same.

I fixed hit sounds in TTW, they don't work in vanilla.

Not sure about weapons.

Do the hit sounds you fixed also account for the player uttering pain grunts? In my vanilla new vegas profile it seems I can only trigger pain sounds when falling from a height. I am also using the YUP patch and the Unofficial Patch NVSE mods in that profile.

Think we could possibly see a load order? I'm on Windows 10, and have no such issue.
