Do you use one of the different frame limiter mode? I get really poor input lag with the default.

I use the framerate limiter without v-sync, v-sync causes massive input lag. I set the sync to fast sync.

Ok, I've made those changes.
Having played more, the problem is still there. Seems particularly evident after an explosion, but even just running around, reload sounds will hitch and then catch up and sometimes guns will become silent and then all sound at the same time.
Anything else I can try?

Try downloading the latest vorbis libs from nexus, the ones we included are a bit outdated and will be in the next update.

Seems to happen when heads explode, whens there are a lot of explosions and it seems to happen after about 30 minutes of playing.

I have basically the same problem after about 30 mins. When performance starts to drop I just save and exit. Not the best solution but works for now.
I started with a completely fresh NV install and used Mod Organizer 2 to install TTW, etc. I can say that New California was much worse with this bug as I could only play for a few minutes or until the first battle with more than a few NPC's. There is a particular battle towards the beginning that slowed to a crawl, single digit FPS complete with terrible sound delay. At least TTW is very playable and amazing so far. (Thanks Roy!)
What are your detailed system specs? Maybe we can spot something in common.
My specs:
6850K on Asus Sabertooth X99 OC to 4.1GHz
16Gb DDR4 2667
EVGA 2080Ti FTW3, Driver 416.81
Intel Optane 900P 480Gb (NV installed here)
Sound Blaster Z (Latest driver)
Windows 10 Pro 1803 (Build 17134)
NV Running max launcher settings at 4K.
NVSE, NVAC, and NVSR with Roy's recommended settings.
I tried a variety of windows compatibility settings but no real change. I've also added an exclusion to windows defender for FalloutNV.exe. No noticeable improvement.
I tried running ALchemy to re-enable EAX thinking it might make a difference. It works great for a minute and then all sound cuts out followed by a CTD soon after.
I agree that it is a problem with this box, doesn't happen on my wife's 2500K/GTX 580 Win7 box.

Sounds like something is terribly wrong with your sound drivers honestly.
