I had that happen to me once. Our respective load orders have very little in common. The only mods I have besides the required ones and some of the optional ttw ones in common with you is the pipboy one and MCM. If I remember correctly, I discovered that, somehow, one of more of my ini files did not have this line right :

SCharGenQuest=001ffff8 . It may also have failed to leave blank sIntroMovie.

Once I made sure all three ini files were consistent that problem went away.

Missing pipboy icons are usually archive invalidation issue or missing texture.

My recollection of the cause of the problem may well be faulty as well. I am sure you looked at the right ini files-the default ini in your main game folder and the two inis in the "my games" folder where saves are stored. It may also have had to do with the pipboy mod but I doubt that since I recall that sort of screen appears when you attempt to launch the game and before the load screen even appears. It appears that you are not using any HuD mod like darnified like I do. I am sorry my recollection is not more clear. I recall chasing a lot of rabbits down a hole before I finally fixed the problem.
