Sorry if anything in the order is misspelled i am unsure how to copy paste using fomm. 

So it is still early in the game and i was able to heal in megaton now when i went to the goul underground and spoke to the doctor to heal the dialog goes through and it fades out but HP bar does move. 

 

I understand that Arwen realism mod is not fully modded for ttw but i am unsure if that is the mod responseible for the mishap.

Until you run the game without Arwen's, we can't 100% confirm anything... unless someone else meets with the same issues and is on a clean, perfectly installed TTW with no other mods.

I just tried this quickly and the Ghoul doctor healed me and also healed my rads when I picked both options.

It is Arwen_med-Tek that is causing this. I don't understand why.
