Difficulty in NV if starting from FO3 (with PN)?

I'm playing with a FO3 start with PN. I'm just wanting some opinions from anyone else with such a setup if the NV content is actually difficult if I don't do that until I'm fairly leveled from the FO3 content (like in the 20s or 30s). I don't want to get to that stuff and then have the combat be a cakewalk.
