I feel like you should get the surgery menu after you go through the train ride the first time. It has been nine years, your character is not going to look the same as they did when they were 19. And, you did get shot in the head, and doc did have to patch you up.

Just use the command Player
Just use the command Player.Showracemenu to change your face and such. I also use a mod that allows me to reset my character because being level 40 at the start of NV makes it pointless for me. If you want the mod here you go

player.showplasticsurgeonmenu should work. It's the same thing as talking with Plinkerton.
