You sure the NCR would recruit the ghouls ? I mean, yes there is that Ghoul ranger squad. However, from what I can gather from the interactions with that one Ghoul Private in Searchlight, the NCR has a sort of stigma when it comes to ghouls. Not as bad as DC, but I doubt the NCR would be actively recruiting Ghouls above other citizens of DC.

I could see it working as such: NCR vs BOS during a battle there is a report retrieved by the NCR that mentions Lyon's faction moving eastward. This is found by a "Mac Arthur or Patton" type that was personally affected by a battle with BOS and wants revenge. This is further compounded with the finding of the working train. The NCR government is reluctant to give permission for such a costly and long term operation due to popularity at home and current problems with the Legion over Hoover Dam. The Brass "officially" denies any operation but unofficially doesn't stop said General from pursuing his vendetta with the hopes of some pre-war spoils from Vault-Tec HQ, L.O.B., Pentagon, etc. Once arrived in DC the NCR recons and discovers UnderWorld. Talking to Willow they learn of the "no love" aspect of the BOS shooting at the ghouls and use this to recruit them to help battle the BOS, added bonus that the SM's leave the ghouls alone allowing the ghouls to operate in the Mall with little hindrance.  The NCR also pays Talon for random  chaos aimed at the BOS. 

Legion has no interest in the East only to the West and the destruction of the NCR so I wouldn't see them go East until there is no West left to conquer.

The only NCR in DC I think would be appropriate would be a small expeditionary force dead in the train station, failing to withstand the pressure from the overwhelming Super Mutant forces present.
