Possible karma bug?
Maybe my karma got so low that it went to "Very Good" after hitting the minimum? I'm slightly knowledgeable about modding, but I'm pretty sure I don't have any mods that change karma, besides the "NoKarmaDCFollowers".
I'm using TTWfixes, but I've read that it causes a lot of problems.

It's no bug. Fallout 3 was
It's no bug. Fallout 3 was stupidly made in terms of karma and is always pushing good karma for the player when killing feral ghouls and stuff like that. If you want to get bad karma you have to go around and steal all the owned junk you can find and kill "good" NPCs.
Basically, in Fallout 3, if you want to play an "evil" character with evil karma, you have to do all the obvious "evil" quest solutions and pick all the "evil" dialogue options. And don't give water to the water beggars or donate to churchs either.
The Contract Killer perk might help, since you get 10 negative karma and some caps for each ear you turn in, in addition to the bad karma you got from killing the NPC. Enslaving people will also help (so it helps becoming a slaver). Once you're a slaver there will be parties of anti-slavery people that will appear to attack you, killing them will give you negative karma and with the contract killer perk you will get even more negative karma, once you become evil karma, you will also be attacked by regulators which also give negative karma for killing them.

Fallout 3 doesn't reward good karma for killing ghouls and raiders that's new vegas. If you play regular Fallout 3 you don't get good karma for killing ghouls that stupidity was done by the creators of New Vegas for whatever reason. OP their are few karma fix mods on the nexus for New Vegas that you can use that fix this problem with the way New Vegas rewards good karma.
