I'm hardly an expert on this, but if it were linked to the bsa's, would it only present on save loads? Or are they all loaded at the same time, as opposed to when the resources are needed?

Unpack all the bsa's for TTW
Unpack all the bsa's for TTW and any mods you have (then remove them) and see if it makes a difference. May want to do that in a separate folder and install them as mods so you can remove them again.
Things like redesigned don't unpack, because egm files need to be in bsa's to load.

Hi, thought I'd chime in with my variables, it might help...
~30 hours of playtime on a ~7.6MB save file, takes ~15 seconds to load into game from the menu.
Mod List/Load Order is attached.
I currently use TTW v2.9.4a & I have TTWOptions loaded but I do not use any of its settings in MCM.
I also use NMC high res texture packs and UHQ ground textures.
I hope this helps.
Edit: Could it be the games INI files? I don't know a person who owns a Gamebryo game who has not modified them in some way...

I mean, probably. But since removing it (tested that a long time ago) doesn't fix the problem for me, it would have to be some sort of bloat added to the save, and I can't think of anything in Interors that would do that (or rather, any reason for anything in Interiors to do that).
Still, It's next on my list so I'll be trying without it soon enough.

A good portion does. It adds a ton of exploring to both wastelands not to mention additions to the player homes and companion options of firing to "home".
Chuck has been on the TTW team forever and interiors is his mod so it's very well integrated with TTW with no issues.
I've used "interiors" since it was first put up on Fallout 3 Nexus and never played a game without it installed.
