So I got the rielly's rangers quest and when i go to try to talk to her, i can't continue because Doctor Barrows ain't acting right. He doesn't access his sound files and when i convince him to wake her up, he just stands by the window and doesn't head over there and get her out of her coma. This problem existed during 2.7, but ignored it and went on with my game. Figured I bring this up and check to see what I could do to fix it.

remove Russell
remove Russell or look @ https://taleoftwowastelands.com/comment/35015#comment-35015 to see how to fix it. There might be another mod conflicting in the same manner but the fix remains the same as for russell.

His problem (while you are correct in the skill requirements) is not that, his problem is missing npc voices and interactions and solution is exactly what I posted in the missing npc thread. Some NV mods break the greeting topic in dialog group, don't ask me how. Even if he had the skills required doc barrow would still be voiceless and broken with russell in LO.
Install Russell for yourself and watch it break fo3 freeform npcs, scavengers, doc barrow and who know who elses voices unless you do the override.
"Russell has nothing to do with TTW" as someone tried to dismiss the issue in that thread is not an explanation - while it's true - it still breaks TTW unless you do the override for the Dialog Topic.

Also breaks random NPCs , Scavs, Hunters, Doctors, etc

Ok well, Barrows still seems
Ok well, Barrows still seems broke. Won't heal addictions or perform medical stuff. Only goes between two spots observing the glowing ones in his little containment. I have uninstalled russel and willow; i'll just reinstall them when i'm in the mojave. The only improvement though is he speaks like a few generic ghoul doctor lines. I can say that the random salvagers in the wasteland now have spoken dialog, even though silus at the lincoln memorial doesn't speak and doesn't move from his spot either.
EDIT: Oh and still doesn't revive Rielly from her coma.

I made the patch that mamelukturbo described, and it fixed all the issues. I had noticed also that Barrows would not heal me or let me start the Reillys Rangers dialogue.

I admit I limited my testing
I admit I limited my testing solely to the voices so the fix wasn't trying to fix doc barrow per se, just him being mute along other npcs. I just assumed it would restore to him the dialogue as well as the functionality, same as it did for the the freeform npcs/scavengers and vendors from the TTW rivet city merchants mod, ie without the fix they were both mute and nonfunctional (no sell/repair).
Perhaps the Russells added additional topic has something to do with Doc Barrow since iirc it was dealing with healing too? Maybe there's more from Russell that needs to be overriden - I only checked the GREETING group. As I mentioned in the other thread, in all the years of modding the fallouts this is the very first time I had to touch Dialog Topic to solve conflict as I've never seen it causing conflict before - maybe this is due to somewhat unique nature of TTW.
