Here's the list. It's public to edit, so feel free to make corrections--and it will need some.
All I did was merge the perk lists from the Fallout Wikia, and removed FO3 duplicates on the assumption that TTW favors the NV versions of perks.
As far as the recommendations go, of course they're subjective. Each column represents perks specific to one kind of skill. I deliberately passed up skillpoint perks, AP perks, carry weight perks, radiation perks, and generally any perks that wind up being a waste in a game that is already pretty easy. The numbers at the bottom help to build a character with preferred perks, keeping in mind that you can only have 25 max.
