True about windows 8 being more ram efficient, but with manufacturer systems there is so much bloat that it is ridiculous. Your CPU is pretty dated, how long have you had it? Do you run any other games/apps that tax the CPU highly? On your graph, is that Fallout RAM usage? Or total system RAM usage?
My CPU is: Intel(R) Core(TM)2 Duo CPU E8400 @ 3.00GHz (2 CPUs), ~3.0GHz
I've had it since I built the machine. A new CPU at this stage would likely require a new motherboard.
It's strange that not even your quad 2.2 is getting to 50% when my duel 3.0 is getting maxed.
I have, thus far not found a game which places this much pressure on my CPU.
The graph represents total system RAM usage, unfortunately I don't know how to divide it up by process, but as you can see, it doesn't seem to be working overly hard.
The E8400 was launched in 2008, have you had it since then? I've never had a processor more than a few years and they usually begin to get unstable by then.
Do you play Skyrim perhaps? Or any other fairly modern game?
If that is total RAM then it is not as likely to be a RAM problem, but I still think Win8 might be to blame. You could set up a dual boot to test that, but that's a lot of work.

I image I got the CPU around that time, if not it would have been within 6-8 months of that, really can't remember when I built the PC now.
I do indeed play skyrim. On max settings. With a shit-load of mods (that's the technical term) and I've never encountered a single issue aside from the occasional blip in the system (which is once in a blue moon.)
Hence why you can understand that TTW is confusing me, does it require a higher spec machine that something newer like skyrim?
One thing to remember is that both Fallout 3/NV and Skyrim were built for the XBox 360
So while the improvement is drastic, the system requirements did not increase quite as drastically because of hardware constraints, they had to work smarter, not harder.
That is the primary reason why I still think RAM (even though all the evidence points away...) is because TTW has twice as much stuff to load into RAM as FNV. It doesn't load it all at once, of course, but the load is still much greater. Full circle, though, your hardware chart doesn't show exceptional RAM usage, so I am at a loss.
Based on what you've told us, I want to say it is some bizarre audio issue with windows 8. This is a total shot in the dark, but try installing this http://xiph.org/dshow/ and see if that helps.

Did you try to run FalloutNV Launcher and/or specifics loaders in "windows 7 compatibility mode" ? http://www.sevenforums.com/tutorials/316-compatibility-mode.html
Did you installed FalloutNV in the "Program Files" folder? If you did, it could be the problem. Windows's "UAC" is the guilty...
Are you using the "Onboard Audio" or a real sound card and/or some program like THX Studio Pro or something ? I cannot explain in a few words what is the real problem with Windows and "Onboard Audio" but try this in google : win audio crash : Like ALOT of peole, i do had myself some audio related crashes with some games and THX Studio Pro. (MSI z77MPower- i5 3570K- Onboard Audio- 8GB GSkill- HD 7870-2GB)
Did you tweaked Windows in anyway? and did you tweaked Fallout.ini or "over-tweaked" it maybe? I am not saying that you did, iam just asking ;)
So it can run on Windows 8 and we've already established that my RAM isn't being pushed hard enough to warrant an error. Looks like a CPU problem then. In which case, I won't be able to play TTW again until I replace my motherboard and CPU, which won't be happening anytime soon. Sad times.
I don`t know if this makes a difference or not but I use win7 and while I was browsing the steam sales the other day i saw fallout3 on it. I clicked the store page just to have a look and it had a big banner for me to see saying that fallout 3 isn`t optimised to run on my OS. I thought maybe that`s why the game can sometimes run sluggish, who knows. I can play skyrim fine on it`s default settings and that game runs so smooth it brings tears to my eyes, but when I play a fallout game on the older engine it stutters, chugs, and can just make the game unenjoyable sometimes. It`s kind of strange because the game can sometimes get these odd bursts of speed where you will run faster and it`s super smooth, then you`ll get hit with stutter and chop and slowdowns out of nowhere.
