Question about starting TTW

Hello! I am new to the forum. I wanted to try installing and playing TTW, but I don't quite understand how it works, in terms of story. Which wasteland do you start in? And does the Lone Wanderer simply become the Courier (or vice versa)?

You start in vault 101 and when you decide to take the train to Mojave you wake up in doc mitchels the first time then you can go back and forth whenever.

I have a question, if you are playing story wise and don't take the train to New Vegas from DC. Do you go to New Vegas at the end of the Fallout 3 story line or does the game end?

In my mind you start the events in Fallout 3 then after all that you become the Courier and start the events from New Vegas. But I am not sure what happens at the end of Fallout 3?
