So is there any reason why the developers didn't flag every REF as Persistent Reference or was it just an oversight, respectively they weren't aware of this problem?
Are there any negative side effect of flagging everything as Persistent Reference (save game bloat?)?

Console memory limits, it's fine to do on PC. We've done so in 3.3, although it can have a few unforeseen circumstances.

nothing major, so far I've seen radroaches go through doors they shouldn't, even though they are marked "can't use doors"

Bethesda still has problems with that. People constantly complain about Dogmeat opening doors in their settlements.
It's not quite the same thing. Closer would be bandits who won't chase you past their territory. That also prevents them from following you through doors. So there are mods that change that flag to allow enemies to chase you past the edge of their territory, which can mean going through doors.

It's unbelievable how much this bug is pissing me off. Are we absolutely certain that this bug also appears in Vanilla Fallout New Vegas / Fallout 3 without any mods

I noticed it again in the Turtledove Detention Camp where all Robot Army spawns and all turrets BUT ONE were missing. It's absolutely weird. Why would most of the spawns, but that one turret spawn not work? Doesn't seem to be a cell issue either, otherwise none of them would have spawned since all of them were in the same cell.
