WARNING: Improperly messing with the registry will mess with your computer, so do this at your own risk. Make a back up as well.
So in case you didn't know, in task manager there is something called a task priority. It is what tells the computer how much resources should be utilized for any application. Usually you can change it from Very low, Low, Normal, Above average, High, and realtime.
Sometimes however, some games do not like that. Fallout New Vegas for example likes the low priority, and you can change it, but it defaults back to low. I however, have found a alternative to force Fallout New Vegas to go on high priority, and it's actually quite easy:
1. Press Windows Key+R
2. Type "Regedit" (Ignore quotations)
3. Go to "Computer\HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows NT\CurrentVersion\Image File Execution Options"
4. Make a new key called "FalloutNV.exe"
5. Make another key inside the new key called "PerfOptions"
6. Make a DWORD inside the PerfOptions key called "CpuPriorityClass"
7. Double click on it, and change the value to 3.
Now do this 2 times over for 2 new keys. Go back to the "Image File Excecution Options" key.
Make 2 new keys inside of it called "FalloutNVLauncher.exe", and "nvse_loader.exe"
Repeat steps 5-7 inside both new keys.
8. Restart computer.
There you go! I noticed quite a lot less load stutters from this. Hope it helps you!

Have you tried using an ENB with EnableUnsafeMemoryHacks=true ? It also greatly reduces stutter, as it prevents Direct3D 9 from mirroring graphical resources into both RAM and VRAM, instead functioning like Direct3D 11 by just loading resources into vRAM. The only draw back is when you play in exclusive full-screen mode (not borderless windowed, which works fine with it), the game will crash when you alt tab.

ENB crashes my game, also the game crashes with alt tab, whether borderless, or fullscreen (For me)
