This happens with and without mods.
I have tried stutter remover and it still happens.

This can actually be quite normal. In certain spots in the FO3 world this is normal. Some spots in the wasteland or bigger cities like Megaton. In the original game it was even worse. As to why this happens, no idea. I guess the engine can't handle the load at times. Not too unusual for Bethesda games though. FNV ran a lot better. No idea why. Maybe Obsidian figured out how to optimize the world space. But I think compared to FO3, FNV also has less details and things in the world.

To be honest I got used to it. It very rarely goes below 50FPS, except in heavy fights to 40 maybe. Good enough for me. Some mods help a bit but even they only improve it by a few FPS. Might also be because they are not exactly optimized for the FO3 DC world.

But it in the end also depends on your resolution and mods used. Especially going higher than 1080p kills the engine. I had huge bottleneck framedrops with the engine at 4K, etc.
