Great news: I was able to successfully initialize all of the arwen tweaks and they seem to work pretty well in TTW, but I did not do too much of the testing.
Now Im going to add medical resupply script to the DC medical vendors.

taro8, did you take the train to NV? I hacked up Arwen's mod myself, but on reaching Docs my character wouldn't get off the table. Now I wouldn't be surprised if this was from my lack of talent or another mod. I just sent Arwen an Email asking for help.

Nope, I just exited the vault 101.
I would like to release the modified esp's, but Arwen does not allow that.

Well, maybe she will answer. We could post a request in her forum.

Do we need the author's permission to share a working version of arwen's realism tweak?

if it is a patch, no, if you modified the file itself, yes.

Here are the current guidelines, from this thread: https://taleoftwowastelands.com/content/nexus-upload-guidelines  Normally we are pretty lax about conversions; it's up to the person who converts to contact them and/or then bear the wrath if they don't bother and the author complains.
...
If you want to use someone else's mod as a starting point, or use their mod's assets in your mod:
	-
Read their mod's description page carefully.  The author may have stated their permissions for using their work there.

			-
It is important to note that while Nexus has implemented a "Credits and Distribution Permission System", most mod authors have not yet used it.  Still it is worth checking, simply by clicking the "Perms" button on a mod's page.  It you see a red warning at the top of the box "This user has not updated their distribution permissions since the system was implemented" then you can disregard the information there, as it is still set to Nexus defaults, and doesn't provide any help for determining permissions.
			-
You may also want to check the mod's "Posts" and "Forums" comments sections, where the author may have given permission to others in the past.

	-
If you can't find anything on the mod's page, send them a PM/Email asking for permission, and then wait a week or so for a response.

			-
Because many Fallout 3 and Fallout New Vegas mods have been abandoned by their authors, you may never receive a response.

	-
If you don't receive a response, send them another PM/Email explaining that you have proceeded without their blessing and that they can contact you at any time if they wish for you to stop.

			-
Make sure you are polite and that you let them know that their work will be credited prominently (and linked to) from your mod's page.

	-
If you do not get a reply back, be aware that the Nexus typically does not allow you to re-use other people's work without explicit consent (as opposed to their implicit consent, which you have, since the author did not respond to your request).  However, this "explicit consent" rule is only enforced on the Nexus if someone complains or notices, and many popular mods currently available on the Nexus use content from other mods with only implicit consent from the original authors.

			-
You are always welcome to post your mod here in our Mod Releases Forum in either case.

	-
When you're done, credit ALL authors who contributed to your mod and link back to the original mods that you used, even if they say they don't need to be credited.  Give credit where it is due.

			Hi so much reaction!
 https://taleoftwowastelands.com/content/nexus-upload-guidelines
the link don't seems to be working.
To make it short, I am trying to contact the author's to have the right authorization.
There is only two scripts to change a little bit to make it work. If I do not get any answers from the author, can I share a patch to make it work without touching the original mod?
Thanks

Yes, just copy whatever you changed into a new plugin that requires the original as a patch.
