Okay, so my game constantly crashes at random times, sometimes 15 minutes into the game, sometimes an hour into the game. When the game crashes I either get: Stuck on the pinwheel of death loading screen, "Fallout NV has stopped working" or the game will just freeze and I have to sign out of windows and sign back in fro the game to close :D
I have uninstalled many of my mods to see if they are the problem, but to no avail. I'll add my load order to this post, if it is all over the place,well I'm not an expert at FO modding just yet!

Ok might not be the same
Ok might not be the same issue as yours but I will at times have the game act like it is frozen. First couple of times I encountered this I cont/alt/del to get out. But then I decided that when it happened the next time that I would just sit there a bit longer and see what happened. Well after waiting a good minute or so the game unfroze and I was able to continue ok. Not sure what causes this but at least I didn't have to blow out of the game.

Sometimes it's NVAC. Most of the crashes that NVAC "fixes" are improperly aligned data passed to SSE opcodes. The exception handler will handle it manually to deal with the alignment issue, but that makes the operation very slow were the game expects it to be very fast. If a lot of these instructions occur over a large enough block of misaligned data, the game will "pause" while NVAC slowly churns through the data. It's better than simply crashing.

I've seen this problem myself. It only happens when I have another window active instead of FNV while it's loading (like I alt-tabbed) or if I run in windowed mode with certain other windows open in the background. FNV will continue loading data and windows thinks it's locked up if you try to make the FNV window the active window while it's loading data. It'll also do it if you have Task Manager open in the background. If that's the sort of behavior you are experiencing it sounds like a different problem to the OP.

Hmm? I don't use CASM. Maybe I should... I'll get the latest copy.
EDIT: The latest copy just got updated Aug 28, so it's quite new. Works pretty well, but I have to turn off the Autosave on Reach Location. It's too aggressive - it makes the game stutter like crazy. I guess the algorithm they use to see if you've reached a different location takes too much CPU time. Anywho, with that disabled, it runs like normal, and now I don't have to worry about spamming the quicksave. I also like how you just hit F4 for a new full save.

I'll have to verify but generally when I run TTW or any other FO game I try to do so without any other windows open. Sometimes I might forget to close out of my browser but generally I do remember to close that before running the games. Also this only happens with TTW. I've never had it happen when running FO4 and I don't remember it ever happening when I played just FONV or FO3. I do use CASM (latest version) but it did it even before I installed it. I actually decided to install it because I thought the game was actually crashing and wanted to be sure that I had a recent save to reload. So I can pretty much rule out CASM as being the issue.
