Not sure if this helps, I've also heard of the game crashing at the very start when playing audio. If it applies to the vegas game engine it might have something to do with setting the audio output to 44.1 instead of 48khz. 48khz, if I remember correctly was causing the problem.

On both FO3 and FNV, I
On both FO3 and FNV, I deleted all local files (not using steam), then verified the cache with Steam. I did use HUD mods on both games before doing that, but shouldn't those mods be undone?
I just launched vanilla FNV, no crash. I'll try not loading the optional files, see how that goes down.

uh, yeah, optional files needs MCM, and so does a huge number of other mods

But would it cause the type of CTDs he's having? Yes MCM is required but wouldn't it just mean that you would not be able to select and make changes those options. Maybe I'm wrong but I didn't think by not having MCM installed would cause CTDs especially during startup. But it wouldn't hurt installing it just the same and see I guess...
