Is there any way to clean up a bloated savegame? My current TTW-save is at 21mb and it starts to seriously affect my game performance. Can I still fix it or do I just have to deal with it?

With TTW there's just a LOT
With TTW there's just a LOT of stuff that has to be saved. Besides two game's worth of clutter and misc objects to remember you taking or bumping in to, there's also every cell you've visited, quests and their associated scripts, and objects and all their scripts, as well as the locations and states of a lot of NPCs and other various creatures and things you come across. Doors you've opened, containers you've looted and other stuff. Large saves are just something you kind of have to deal with. You can keep them smaller by doing certain things, but those things make the game not as much fun, so probably not worth it.
That being said, there is a tool for cleaning up save files that will remove all that, but it also restores the items you've taken back to the game. As it is a save modification, I do not endorse its use simply because it can mess things up. Make backups.
